Medical clearance is documentation from a healthcare provider confirming an individual is safe and stable for sober living admission. It serves as proof of sobriety when a resident cannot provide a clean UA or alcohol test.
Arizona Department of Health Services (AzDHS) requires licensed sober living homes to verify sobriety for all new admissions. Medical clearance protects resident safety and ensures individuals are appropriate for a recovery-focused environment.
Tell them you are being admitted into a sober living home, cannot provide a clean drug or alcohol test, and need medical clearance to confirm you are safe for admission.
Accepted paperwork includes discharge documents from detox, residential, or outpatient treatment, or medical clearance from a primary care provider, medical doctor, or urgent care clinic.

If you complete treatment at a detox, residential program, or outpatient facility, simply bring your discharge paperwork—this serves as your medical clearance.

You can also get medical clearance from your primary care provider or a walk-in clinic like MyDrNow—just bring the signed clearance or visit summary showing you’re safe to admit.

Medical clearance can also be obtained at an urgent care clinic, where a healthcare provider can confirm you’re stable and safe for sober living admission.
